Who Is Eligible?

All living individuals in the United States who were sent a notice informing them that their private information “may have been impacted” are automatically members of the Settlement Class—unless they fall into one of the limited exclusions (e.g., Yale New Haven officers, the presiding judge, or certain government entities).

What Can You Receive?

1. Cash Payment A – Documented Losses (up to $5,000)

  • Unreimbursed fraud or identity-theft losses
  • Credit-freeze or unfreeze fees
  • Out-of-pocket expenses such as postage, notary, copying, mileage, telephone charges

You must provide receipts or other reasonable documentation. Personal affidavits alone are not enough.

2. Cash Payment B – Alternate Cash (≈$100)

If you prefer not to gather documentation, you may choose a flat alternate payment estimated at $100 (subject to pro-rata increase or decrease depending on claims volume).

3. Two Years of Free Medical Data Monitoring

All claimants—whether choosing Payment A or B—can also enroll in two additional years of medical data monitoring services at no cost.
