Who Is Eligible?

  • UnitedHealthcare of North Carolina or UnitedHealthcare Insurance Co. member at the time of service.
  • Received treatment from a provider outside the UnitedHealthcare network.
  • Paid the provider more than the plan-required copayment, coinsurance, or deductible.
  • Service took place between Jan. 1, 2016 and Dec. 31, 2021 (or the last renewal date of a policy active on Dec. 31, 2021).

Claims may also be filed on behalf of deceased, incapacitated, or minor members with proper legal documentation.

Potential Payout

Eligible members will be reimbursed for the difference between what they paid and what their UnitedHealthcare plan required, plus applicable interest.

  1. Online: Visit uhcncvoluntarysettlement.com/Login and enter the unique ID and PIN from your settlement letter.
  2. Mail: Print and complete the claim form, then send it to:

Required Documentation

  • Itemized provider bill showing patient name, service codes/descriptions, dates, and amounts charged.
