Who Is Eligible?

  • All U.S. residents who received Sunflower Medical Group’s data-breach notice stating their personally identifiable or protected health information was potentially accessed.
  • Exclusions: the presiding judge and family, Sunflower employees who did not receive a notice, all government entities, and anyone who opts out.

What You Can Receive

  • Cash Option 1 – Documented Losses: up to $5,000 reimbursement for out-of-pocket losses (identity-theft expenses, credit-monitoring fees, ID replacement, postage, etc.) incurred between 12/15/24 and 3/26/26. Proof required.
  • Cash Option 2 – Alternative Payment: $10 flat payment, no documentation required.

Note: Cash payments share a $300,000 aggregate cap and may be reduced pro rata if claims exceed that amount.

How to File a Claim

  1. Visit the official settlement site: sunflowersettlement.com and select “Submit a Claim.”
  2. Complete the online form—or print, complete, and mail the PDF claim form to the Settlement Administrator.
  3. For documented-loss claims, upload or mail receipts, bank statements, or other proof.
  4. Submit online by 11:59 p.m. CT or ensure the mailed form is postmarked no later than March 26 2026.
