Pacific Guardian Life Ins. Cybersecurity Incident

If you received a notice that your personal information was exposed during the August 25 2023 data breach at Pacific Guardian Life Insurance Company, Ltd., you may claim up to $2,000 in reimbursement, cash payments, and two years of free credit monitoring. Submit your claim by December 21 2025.

Who Is Eligible?

All individuals who were notified that their private information—such as names, Social Security numbers, financial or medical data—was potentially compromised in the August 2023 cybersecurity incident.

Settlement Benefits

  • Expense Reimbursement: Up to $2,000 for documented out-of-pocket costs (credit reports, bank fees, postage, etc.).
  • Alternative Cash Payment: Up to $50 without documentation. California residents may receive an extra $20 under CMIA.
  • Credit Monitoring: Two years of free credit monitoring and identity-theft protection.

All payments and services are subject to a $2 million aggregate cap; amounts may be prorated if claims exceed this limit.
