Mindpath Health Lowrey Data Breach Settlement

If you received a Mindpath Health data-breach notice in or around January 2023—or received Mindpath services before August 2022—you may be entitled to cash payments and other relief from a $3.5 million class action settlement. Submit your claim online or by mail before the January 5 2026 deadline.

Who is eligible?

  • All individuals Mindpath identified as potentially impacted by the March 2022 and/or July 2022 Microsoft O365 email intrusions.
  • Anyone who was mailed a data-breach notice in January 2023.
  • California residents at the time of either incident form a “California Settlement Subclass.”
  • Excluded: the presiding judge and immediate family, judicial staff, Mindpath officers/directors, and anyone who validly opts out.

Available settlement benefits

  • Automatic $50 payment for California Settlement Subclass members (amount may be adjusted).
  • Additional cash and other relief for all class members who submit a valid claim form.

Key deadlines

  • Claim, Opt-Out, or Object: January 5 2026
  • Final Approval Hearing: February 19 2026 at 10:00 a.m. PST (Los Angeles County Superior Court)

Your legal options

  • Remain in the class & claim benefits – You’ll receive the settlement relief and give up separate claims.
  • Opt out – Keep your right to sue Mindpath individually, but receive no settlement benefits.
  • Object – Tell the Court why you dislike the settlement but keep the chance to claim benefits if it’s approved.
  • Do nothing – You give up the right to sue but get no benefits (unless you’re in the automatic $50 California group).

Case background

The lawsuit—Mindpath Health Lowrey, Case No. 24STCV30135, Superior Court of California, County of Los Angeles—alleges that cyber-intruders accessed Mindpath’s Microsoft Office 365 email accounts in March and July 2022, exposing patients’ personally identifiable and protected health information. Mindpath denies any wrongdoing, and the settlement does not admit liability. The parties agreed to resolve all claims for $3.5 million, subject to Court approval.

What is the Mindpath Health Lowrey settlement about?
