Who Is Eligible?

Anyone residing in the United States whose personally identifiable information (PII) or protected health information (PHI) was accessed or acquired without authorization during GWC’s January 5, 2021 data security incident.

Potential Settlement Benefits

  • Alternative Cash Payment: Estimated $750 (subject to pro rata adjustment).
  • Documented Loss Reimbursement: Up to $5,000 for out-of-pocket expenses fairly traceable to the breach (in lieu of the cash payment).

How to File a Claim

  1. Locate the unique Notice ID and Confirmation Code found on your postcard notice. If you never received one, call 1-888-995-4054.
  2. Complete the online claim form at the official settlement website or mail a paper form postmarked by April 26, 2026 to the Settlement Administrator.
  3. Attach documentation for any out-of-pocket loss reimbursement request. Redact unrelated transactions and all but the last four digits of any account number.
