How to File a Claim
- Locate the unique Notice ID and Confirmation Code found on your postcard notice. If you never received one, call 1-888-995-4054.
- Complete the online claim form at the official settlement website or mail a paper form postmarked by April 26, 2026 to the Settlement Administrator.
- Attach documentation for any out-of-pocket loss reimbursement request. Redact unrelated transactions and all but the last four digits of any account number.
