Who Is Included?

You are automatically part of the Settlement Class if Trinity Health mailed you a data-breach notice dated April 6 2021 to a California address. No further action is needed to remain in the class, but you must submit a claim to receive money.

What You Can Receive

1. Pro-Rata Cash Payment

Every class member who files a valid claim will share equally in the remaining settlement fund after out-of-pocket claims are paid. Current projections:

  • 5% claim rate → about $231 per claimant
  • 10% claim rate → about $115 per claimant
  • 100% claim rate → about $11 per claimant

2. Up to $1,000 for Documented Losses

You may also request reimbursement—up to $1,000—for expenses fairly traceable to the breach, including:

  • Fraudulent charges
  • Credit-monitoring fees
  • Bank fees or overdraft charges
  • Professional fees (e.g., accountants, lawyers) incurred to address identity theft

  3. Submit online or mail to the Settlement Administrator by January 19, 2026.
